Submits the completed Health Benefits Enrollment form and DD214 to DBM

Process Details

Employee sends a letter of return notification with a copy of the DD214. The Benefits Analyst will send the Employee the Health Benefits Enrollment form to complete.

 

Administrative Official reviews the letter of return and DD214 then submits it to Employee Services Manager/ Analyst.

 

Employee Services Manager/ Analyst reviews the letter of return and DD214 and enters the Military Leave into the system.

 

Benefits Analyst reviews the letter of return and DD214 and sends the Employee Health Benefits Enrollment Forms. The Employee will complete the form, the Employee Services Manager/ Analyst then submits the Health Benefits Enrollment form and DD214 to DBM.
